Interested in volunteering? then it's as simple as 1, 2.... 3!

Step 1. Download, print and complete the Release Form: ‘Release for the Special Needs Soccer Program’. (Volunteers under 16 require parent’s approval and signature.)

Step 2. Once the Release Form is completed send an email to: specialneeds@u-msc.com and advise the Program Manager of your intentions to become a mentor. Briefly explain why you would like to be a mentor. A reply will be returned to you shortly afterwards. Once your email is processed and approved you will be asked to continue Step 3.

Step 3. All Club volunteers must complete a ‘Vulnerable Screening Request Form’, which can be picked up at the UMSC club office at 7700 Kennedy Road, Unionville. Instructions on submitting the form to the York Regional Police Headquarters is found on the back of the form.
