Registration Form (down
load from here or pick one up at the club office)
and submit it using one of the 3 following ways:

Method of payment:
Cheque: Make cheque payable to
UMSC. Attach cheque to registration form. Please note that there
is a $25 charge for a NSF cheque.
Cash:
Cash payments can only be
made at the soccer club office. Please ensure you receive a receipt
for each cash payment.
Visa:
Visa payments can
be made at the club or via phone.
Refund Policy:
Registration must include full payment
